Confidential Reporting Policy & Procedure
Omni Air International’s Aviation Security Action Program (ASAP) is a system available to all cabin crew, flight crew, dispatchers, and mechanics through which they can report operational or maintenance issues related to aviation safety. These reports can include instances of noncompliance and will not result in disciplinary action on the person reporting. Each ASAP report is completely confidential. The information in your report will be de-identified before review by the Event Review Committee, consisting of the ASAP manager, representatives from the FAA, and other department members who can provide more information or expertise on the instance.
The Event Review Committee (ERC) will meet periodically to identify actual or potential problems from the information contained in the report and propose solutions for those problems. Your report is especially important because the ERC will review summary data relative to all ASAP reports to identify trends in reporting. With the help of you and other crew members submitting similar reports, we can acknowledge when a repeating issue requires a new solution.
Initial ASAP reports can be filed on the CrewNet SharePoint or by emailing asap@oai.aero. The initial report must include:
- The date and time (UTC) that you are filing the report
- The date and time (UTC) when the event occurred or when you became aware of a potential violation of regulation or company policies, or both
- A brief narrative of the event, where and when it occurred, and who was involved
- A brief narrative outlining why the event occurred
While ASAP reports are confidential, they are not anonymous. If you want to file an anonymous safety report, you can use the Safety and Security Report form on CrewNet. However, if your report is anonymous then we will not be able to ask for more information or provide feedback.
If you notice an issue that could lead to hazardous conditions, an increase in risk, or potential noncompliance, do not hesitate to file an ASAP or Safety and Security report. The goal of our confidential reporting programs is to gather information in a non-punitive manner to ensure that we are constantly working towards safer operations. By reporting, you are contributing to the strength of our system and defining the unparalleled safety standard that Omni provides.
